Outpatient Self-Management Diabetes Education
Our Services
Whether you are new to diabetes or you’ve been trying to manage your diabetes for a while, the Outpatient Self-Management Diabetes Education Program at Mary Black offers individual and small group instruction in all aspects of diabetes self-care.
Basic diabetes education is available to all who are new to diabetes or who need information on:
- What type of diabetes you have
- What blood sugar levels are acceptable
- What can be eaten
- The impact of activity on blood sugar levels
- How diabetes pills work
- How to check blood sugar levels
- What to do for high or low blood sugars
- How to lower risks of diabetes problems
- Tests needed on a routine basis
Education is available for individuals whose diabetes is affected by changing work shifts; interested in intensive insulin management or insulin pump therapy; pre-pregnancy diabetes recommendations; and diabetes management during pregnancy.
Our team can assist you with meal planning using carbohydrate counting to give you a more flexible method of eating and the ability to control portion sizes.
All patients are assessed for individual needs. Individual and small group counseling is a unique aspect of our program.
Our Qualifications
Mary Black's diabetes educators have extensive training and experience in diabetes management and teaching principles, and are committed to providing a caring environment to encourage learning about diabetes.
Our team has received the American Diabetes Association Education (ADA) Recognition Certificate for a quality diabetes self-management education program. ADA believes this program offers high-quality diabetes self-management education that is an essential component of effective diabetes treatment. The association’s Education Recognition Certificate assures that educational programs meet the National Standards for Diabetes Self-Management Education Programs.
Enrollment
A physician’s referral is required for participation in the program. We accept physician referrals by phone or fax. We will contact you to set a time for your first appointment.
We will provide billing services. Your physician’s office can help you get the proper approval from your insurance carrier or you may call your insurance company to inquire. For those not covered by insurance, monthly payments are available.
