Mary Black Health System is now affiliated with Community Health Systems (CHS), and we’re excited about the advantages and opportunities this new affiliation brings. Our goal is to continuously improve services provided to our community. In an effort to achieve this goal, we are transitioning our Seniority Program to a Senior Circle Chapter.
In 1999, CHS developed a program to support healthy and active lifestyles for adults age 50 and better -- Senior Circle Association. This valued program now includes 69 chapters serving over 56,000 members from coast to coast -- with new members joining daily. As of February 28, 2008, MBHS became the proud sponsor of a Senior Circle chapter. You are invited to be members, by completing and returning the application. Membership at this chapter will offer significant advantages:
· Free meal for spouse or caregiver each day a member is hospitalized
· Bi-monthly chapter newsletter “Circle News”
· Monthly health talks by physicians and other health professionals
· Health fairs & screenings
· Exercise programs, social events and travel opportunities
· Vision care (EyeMed) and prescription (ScriptSave) discount cards
· Quarterly Senior Circle national magazine, “Inside Circle”
· Personal emergency response system & home monitoring discounts (ADT)
· Free copying, faxing, notary service and insurance claims filing assistance
· Discounts at area businesses
· 40 % discount in hospital cafeteria
· All these benefits are available at all Senior Circle Chapters across the country (great for seniors who travel!)
In order to provide the expanded list of services, the new payment plan is as follows:
(1) Person, one year: $15
(1) Person, two years: $27
(2) Persons in the same household, one-year: $27
(2) Persons in the same household, two-years: $51
