Benefits at Mary Black

Health Insurance
Full and part-time employees. Coverage is effective on the first day of the month following two months of employment. Eligible employees may choose from two PPO plans with $500 and $1500 deductibles. 100% coverage with no deductible for covered expenses if you choose Mary Black Memorial Hospital.

Dental Insurance
Full and part-time employees. Coverage is effective on the first day of the month following two months of employment. The dental PPO has annual deductibles of $75 per person, $150 per family.

Vision Insurance
Full and part-time employees. Coverage is effective on the first day of the month following two months of employment. In network, eye exams are covered at 100% with a $10 co-pay. Eyeglass lens are paid at 100% with $10 co-pay every 12 months. Frames are covered at 100% every 24 months.

Employee Life Insurance
Full and part-time employees. Coverage is effective on the first day of the month following two months of employment. Hospital pays for life insurance on employee with coverage level of 1x annual pay. Part-time employees may purchase life coverage. Employees may purchase additional coverage ranging from 2x annual pay to 5x annual pay.

Dependent Life Insurance
Full and part-time employees. Coverage is effective on the first day of the month following two months of employment. Employees may select from coverage levels of: $5,000 per dependent, $10,000 per dependent or $25,000 spouse/$10,000 child.

Voluntary AD&D Insurance
Full and part-time employees. Coverage is effective on the first day of the month following two months of employment. Benefit levels available are $25,000, $50,000, $100,000, $250,000 and $500,000.

Long-term Disability
Full and part-time employees. Coverage is effective on the first day of the month following two months of employment. Eligible employees may choose 60% of predisability pay.

Short-term Disability
Full and part-time employees. Coverage is effective on the first day of the month following two months of employment. Eligible employees may choose 60% of predisability pay.

Health Care and Day Care Spending Accounts
Full and part-time employees. Coverage is effective on the first day of the month following two months of employment.

FutureShare
Eligible to participate on the first of the month following two months of service. Matching contributions: Company matches 50% of the first 3% of pay you contribute each pay period. You may contribute 1%-50% of pay (13% if highly paid) on a pre-tax basis. Company contributions (matching and retirement) are subject to vesting schedule.

Retirement contribution: Based on years of service. 

Tuition Assistance is limited to:

A maximum of $2,500 per semester for courses at approved nursing schools

A maximum of $2,500 per academic year (Sept.-August ) for all courses.

Paid Time Off (PTO)
Full-time and part-time employees. PTO is accrued each pay period based on years of service and employment status (full-time and part-time). This benefit combines traditional vacation, holiday and sick leave programs. Maximum accrual is 2x annual accrual.

Extended Illness Bank (EIB)
Full-time and part-time employees following 90 days of service. Full-time employees accrue 10 days annually or 3.08 hours/pay period. EIB may be used for personal illness after the third day of work missed per occurrence. Maximum accrual is 870 hours.

Bereavement Leave
Full-time and part-time employees. Up to three days paid in event of the death of an immediate family member.

Jury Duty
Full-time and part-time employees. Pay granted for missed shifts of normally scheduled work.

Cafeteria Discount
All employees. 40% discount if you wear your name tag.
